HOW TO START HOME HEALTH SERVICES?
Step 1: Talk to your doctor
Ask your doctor for a home health referral.

Step 2: Choose a Home Health Agency
Once a home health referral is ready, ask your doctor to send the home health referral to A1 Home Health or any home health provider of YOUR choice

Step 3: Assessment & Care Plan
A1 Home Health will coordinate the evaluation visit with you and our nurse, then develop a personalized plan of care tailored to your needs.

Step 4: Begin Care
Once admitted, services will begin at home, care home, or assisted living facility, including nursing, therapy

HOW TO START HOSPICE?

Step 1: Talk to Your Doctor
Ask for a hospice referral.

Step 2: Hospice Provider
Once there’s a hospice referral, ask your doctor to send the referral to A1 Hospice, or any hospice provider of YOUR choice

Step 3: Schedule an Assessment
Once the referral has been received by A1 Hospice, a hospice nurse will visit to evaluate needs and create a personalized comfort care plan.

Step 4: Begin Care
Once documents are signed, and the patient is determined qualified for hospice care services, the team coordinates visits, medical equipment, medical supplies, and medication delivery to the patient’s home.
